Many serious accidents have occurred because a machine was started when it was supposed to be completely shut off for maintenance or repair. These incidents are typically the result of energy that has not been released properly. Lock-out tag-out (LOTO) systems can prevent these accidents by making sure that the machines remain off until an authorized person releases them.
The LOTO system consists of a set of locks, tags and other devices that are used to isolate, secure or block the machine from electrical sources. All "affected" and "authorized" personnel are required to be trained in the use of these devices. These devices include, but aren't limited, to chains, locks, and padlocks. Each device is unique and labeled, including the date and the time it was used along with the name and title of the person authorized, and the purpose of the lockout/tagout (repairs, maintenance, etc.). It must also be clearly stated that these devices can't be removed until a full LOTO procedure has been followed.
